NaviCare HMO SNP is a Medicare Advantage Special Needs Plan that combines Medicare benefits and MassHealth Standard benefits, including Medicare Part D.
For people with MassHealth Standard alone, NaviCare SCO offers the same benefits as NaviCare HMO SNP.

There is no cost to you. With NaviCare HMO SNP and NaviCare SCO there are no monthly premiums, no copayments or coinsurance.

Care designed just for your needs
Getting the right care at the right time is important for your good health. Based on your health care needs, you may get:
- Personalized in-home care plan with support services and medical care
- Assistance with bathing, dressing, and home meal preparation
- Adult Day Program providing recreational and social activities
NaviCare gives you personalized, complete care led by a Primary Care Team dedicated to you.The Primary Care Team will have access to a centralized electronic record of your up-to-date health, medical and drug information which will make it easier for your Primary Care Team to manage and coordinate your health care. This team works with you, your family and your health care providers to ensure you receive the best possible care when you need it.
Your Primary Care Team will include:

Primary care physician - choose from our network of doctors that are among the region’s most respected. Your current physician may already be part of our network.

Nurse Case Manager - assesses your clinical needs, helps you learn about your conditions and medications, arranges for nursing home or long-term care.

Geriatric Support Services Coordinator - helps arrange for community services such as home care and adult day programs.

Navigator - acts as your single point of contact who helps to organize your services and care.
NaviCare's service area
NaviCare’s service area is Worcester, Middlesex, Suffolk, Norfolk, Plymouth, Barnstable, Bristol, Hampden and Hampshire counties, and a portion of Franklin County. The portion of Franklin County includes: Erving (01344), New Salem (01355), North New Salem (01364), Orange (01364), Warwick (01378), Wendell (01379), and Wendell Depot (01380).

Next steps
NaviCare is a Coordinated Care Plan with a Medicare contract and a contract with the Massachusetts Medicaid program. NaviCare is a voluntary program in association with MassHealth/EOHHS and CMS. Benefits, formulary and pharmacy network may change on January 1 each year. Limitations and restrictions may apply. You must continue to pay your Medicare Part B premium unless the Commonwealth of Massachusetts pays this premium for you.
